The Plattduetsche Home Society has been providing exceptional Senior Care since 1923. In reflecting over the past century, we are humbled by the many residents and families who have trusted us to care for them and their loved ones. We are also extremely proud of our rich history and our place in the legacy of the community.

To honor the thousands of residents who have graced the halls of our facility, the selfless supporters of our organization, our dedicated staff, and our respected community members, we have created “Legacy Lane”.

How to Purchase

Build for the Future & Remember the Past

Legacy Lane is part of a larger beautification project that includes repaving the walkway from the Hempstead Turnpike sidewalk to the entrance steps of the building. The walkway will be bordered by 4” x 8” and 8” x 8” engraved concrete pavers.

Your personalized pavers are available for a donation via a special donor website that can be accessed here.

This is a great opportunity for you to honor and memorialize friends and family while helping PHS continue its mission of providing the best care possible for our cherished residents.
